The 3-part Project Advance pledge form is organized as follows:
If you are bringing your donation to the office:
• Please bring all three parts of the form.
• If paying by credit card, remember to sign in the space marked “Donor Signature.”
Part 1 & Part 2:
• If you are mailing your donation to the parish, please include the top two sections:
• Project Advance Office (Part 1)
• For Parish Record (Part 2)
Part 3:
• Keep the Donor Copy (Part 3) as your temporary receipt.
